New York City Now Taking Online Applications

VitalChek Now Available for Adoptees Born in the City

If you were born in one of the five boroughs of New York City, you’ve been required to mail-in a paper form to request your original birth certificate. That has now changed. New York City is now accepting online application for the original birth certificate.

    If I’ve already mailed in an application and my check has not been cashed should I do an online application?

    Is NYC giving updates online as to the status of our mail in requests? I mailed mine in and would like to know. I’ll follow up with them and report back if no one knows.
