New York State
The New York State Department of Health has launched its online application process. More information about the application process through the state department of health is here, and adoptees report that the online application process is going smoothly. While paper forms are available for mailing, the department strongly encourages using VitalChek. The state health department handles birth certificate requests for all people born in New York, except for those born in New York City.
New York City
New York City is its own vital records district and handles birth certificate requests for people born in any of the five boroughs. This includes original birth certificates of people who were born in one of the boroughs and later adopted. The department now has a form and instructions on its website, as well as answers to common questions about the process. The cost to apply is $15. Update! New York City is now accepting online application for the original birth certificate.
